Appointment Info
Please arrive 5 minutes prior to your scheduled appointment. Make sure you have completed any intake forms (if they were not already emailed to you) and please bring your insurance card.
If you are scheduled for a telehealth visit, you will receive a HIPPA-compliant video call through Doximity from our office number at your scheduled time. The provider will start the visit as soon as they are available.
Accessibility: We’re located on the second floor; accessible by elevator through the courtyard. Free parking on-site. Visits by appointment only, please.
Mask Policy: In this low volume setting, masks are optional. This is subject to change based on CDC recommendations and Whatcom County Infection rates. Your clinician will mask during your visit if that is your preference!
