Our electronic forms capture your information quickly and securely. Your data is not stored on our website, but digitally sent to our electronic medical records. Forms are HIPAA-compliant and create a paperless experience for patients and staff.
Please arrive 5 minutes prior to your scheduled appointment. Make sure you have completed any intake forms (if they were not already emailed to you) and please bring your insurance card and photo identification.
If you are scheduled for a telehealth visit, you will receive a HIPPA-compliant video call through Doximity from our office number at your scheduled time. The provider will start the visit as soon as they are available. If you have questions or technical challenges, please call us at 360-230-4460.
